I received a message to ban STEAM_0:0:19586231 from Gweedo. Apparently the person using the ID was hacking and using other player's names to try to avoid detection. After checking the logs I found the STEAM ID logged as having only connected once, however no names were logged. This proves he was hacking and using a name change script as well to block the logging. Whoever this person was has been permanently banned.
